Current treatments are not that effective. As patients with LBP are often<br \/>\nfound to be proprioception impaired, new proprioception exercises are required.<br \/>\nWhole body vibration (WBV) stimulates proprioceptive receptors on the muscles,<br \/>\ncausing an alternation in muscle recruitment and hence modifying the muscle<br \/>\nstiffness and joint stability. Its potential to improve muscle function and<br \/>\nproprioception has been shown in athletes. Sinusoidal alternative WBV was<br \/>\nshown to be able to relieve pain in patients with LBP after a long term<br \/>\napplication. However, the underlying mechanism has not been investigated.<br \/>\nAnother study has demonstrated that short term WBV had an immediate effect<br \/>\non improving pelvic repositioning ability on normal individuals. As spinal<br \/>\nproprioception facilitates spine positioning and movement and computes selfcoordination<br \/>\nunder both static and dynamic conditions, it is essential to<br \/>\ninvestigate the effect of WBV on lumbo-pelvic stability, coordination and<br \/>\nrepositioning ability to justify whether WBV has a beneficial effect on spinal<br \/>\nproprioception. In this study, the immediate and carryover effect of a 5-min 18Hz<br \/>\nWBV on spinal proprioception were investigated.<br \/>\nThe study was divided into two stages. In the first stage, twenty young<br \/>\nnormal individuals were recruited and the effects of WBV in standing and seated<br \/>\npostures on spinal proprioception were determined and compared. In the second<br \/>\nstage, the effect of WBV on spinal proprioception was evaluated in eight<br \/>\nindividuals with LBP, with age matched with those in the healthy group, in<br \/>\nseated posture. The results were compared with the healthy subjects receiving<br \/>\nWBV in seated posture. In addition to quantifying body alignment, repositioning<br \/>\n3<br \/>\nability of participants before and after WBV, postural stability and phasic<br \/>\nrelationship between the lumbar and pelvis segments were investigated in terms<br \/>\nof maximum reaching distance and lumbo-pelvic coordination using Dynamical<br \/>\nSystems Theory approach. Assessments were conducted before, immediately<br \/>\nafter, 30 minutes after and 1 hour after 5 minutes of WBV (18Hz, 6mm p-p<br \/>\namplitude). Multivariate analysis of variance was used to study the changes of<br \/>\neach variable and LSD criterion was adopted for post-hoc comparisons.<br \/>\nIn the first stage, it was shown that WBV in both standing and seated<br \/>\nposture had significant beneficial effect on maximum reaching distance, dynamic<br \/>\nlumbo-pelvic coordination and repositioning ability without significant group<br \/>\ndifference. Significant carryover effect of WBV was also shown. In the second<br \/>\nstage, immediate improvement of postural control, lumbo-pelvic coordination<br \/>\nand repositioning ability were observed for the LBP group. However, the effect<br \/>\nseemed to be less long-lasting.<br \/>\nThere were limitations in the study. The effect of vibration intensity and<br \/>\nlearning effect have not been fully investigated. As analysis was limited to the<br \/>\nsagittal plane, the effect of WBV on spinal motor control in other planes was not<br \/>\nknown.. In conclusion, 5 minutes of 18 Hz WBV was shown to have significant<br \/>\npositive effect on lumbo-pelvic stability, coordination and repositioning ability<br \/>\nwithout any apparent adverse effect in both normal and LBP individuals. The<br \/>\neffects were more apparent and long-lasting when WBV was applied to healthy<br \/>\nindividuals than patients with LBP. Further clinical study on patients with LBP<br \/>\nundergoing similar or different WBV protocol is recommended to confirm its<br \/>\nclinical application on improving spinal proprioception.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Low back pain (LBP) is a common health problem with a high recurrence rate.
